Insurance
Our staff files your health insurance as a courtesy. To expedite
this process, please provide us with the most current and accurate
insurance information. We participate in Medicare, Medicaid, as
well as various HMO/PPO plans. In addition, we require co-payments
at time of service. Any outstanding balances following payment
by your insurance company come due within 30 days. Balances over
90 days accrue interest at a rate of 12% APR. Please contact our
billing department with any questions during business hours. We
accept cash, checks, MasterCard and VISA.
